Collaboration of Care

Dr. Chabots office has lost their lease and needs to relocate office space.  We are looking for support of the community to bring the St. Elizabeth Medical Center to the Harding Nursing Home building.   We have relocated supplies and have the ability to move offices to provide space for his medical practice.  The Nursing Home and the Medical Center will operate separately with their own entrances.  If you have any questions or concerns feel free to contact me at 315-841-4156

We the undersigned are in full support of Harding Nursing Home and St. Elizabeth working together to ensure the medical center stays within this community.  Having the medical center located at the nursing home is not only advantageous to the community but to the residents of the home as well.
